Model Release & Photo/Video Consent (Minors)

Scope of Consent

By submitting this form, I (the Parent or Legal Guardian) authorise Illumin8 Productions (the Producer) to record, reproduce, and publish photographic images, video footage, and/or audio recordings of the minor named below (the Materials) in connection with the project described.

I confirm that I have legal authority to provide this consent on the minor's behalf.

Permitted Uses

The Materials may be used for the following purposes:

• Primary use: The specific project deliverable(s) as described below (e.g. corporate video, advertisement, promotional content, documentary, editorial).
• Ancillary use: Behind-the-scenes imagery, showreel/portfolio inclusion, case study, or social media promotion of the project, unless I opt out below.

The Materials may be published in any medium, including but not limited to: broadcast television, cinema, online video platforms, websites, social media, print, and digital advertising. This right extends to both domestic and international distribution.

What This Consent Does NOT Permit

• Use of the Materials in a manner that is defamatory, exploitative, pornographic, or materially misleading as to the minor's character.
• Sale or sub-licensing of the raw Materials to a third party unrelated to the named project, without separate written consent.
• Use of the minor's likeness for AI training datasets or generative model fine-tuning (a separate AI Likeness Consent applies if required).
• Any use that could reasonably be considered harmful to the minor's wellbeing, dignity, or reputation.

Editing & Post-Production

I acknowledge that the Materials may be edited, colour graded, cropped, composited, or combined with other footage, graphics, music, or visual effects as part of standard production and post-production workflows. Reasonable creative editing does not require my prior approval.

Duration & Territory

This consent is granted in perpetuity (i.e. without time limit) and applies worldwide, unless a specific limitation is noted below. I understand that upon the minor reaching 18 years of age, they may independently revoke this consent for future use by providing signed written notice to Illumin8 Productions.

Consideration

I acknowledge that the consideration for this release is the minor's participation in the project, and/or such other consideration as has been agreed between the parties. No additional payment is required for the uses described above.

Right to Review

Where feasible, the Producer will provide an opportunity to preview the Materials in context prior to public release. I may request reasonable modifications if the output materially misrepresents the minor's appearance or character, however I acknowledge that final editorial control rests with the Producer and/or the commissioning client.

Revocation

This consent may be revoked for future use only by the parent/guardian (or by the minor upon reaching 18 years of age) by submitting a signed written notice to Illumin8 Productions. Revocation does not apply to Materials already published, distributed, or contractually committed to a client prior to receipt of the notice.

Release of Claims

I release the Producer and any commissioning client from any claim arising from the use of the Materials in accordance with this consent, including claims relating to editing, colour treatment, juxtaposition with other content, or use in composite form, provided such use does not breach the limitations stated above.

Australian Privacy Acknowledgement

I acknowledge that the minor's personal information and image are collected in accordance with the Australian Privacy Principles (APPs) under the Privacy Act 1988 (Cth). I have been informed of:

• The purpose for which the minor's image is being collected (production of the named project and ancillary uses as described).
• That the Materials may be provided to the commissioning client and/or third-party distributors as required to deliver the project.
• My right to access and request correction of personal information held by Illumin8 Productions on the minor's behalf.

Safeguards for Minors

Illumin8 Productions is committed to the safety and wellbeing of all minors involved in our productions. The Producer will:

• Ensure a parent, guardian, or approved chaperone is present on set at all times during the minor's participation.
• Comply with all applicable Queensland and Australian laws relating to the employment and participation of children in media production.
• Take reasonable steps to ensure the minor's participation does not interfere with their education or wellbeing.

Parent/Guardian Declaration

I confirm that:

• I am the parent or legal guardian of the minor named above.
• The minor is under 18 years of age.
• I have the legal authority to provide consent on the minor's behalf.
• I have explained to the minor (in an age-appropriate manner) the nature of their participation and how the Materials may be used.
• I give this consent freely, voluntarily, and without coercion.
